At Sprout Health New Jersey, we are dedicated to providing treatment programs that are proven to work. Our programs address mental health issues, substance abuse disorders, and co-occurring conditions. We also accept referrals from other providers and healthcare organizations. Our dedicated staff helps to facilitate a seamless transition to our facility.
All the necessary documentation and medical information are gathered quickly and efficiently for each patient. As such, our referral coordinators can better provide an individualized treatment plan for each client. Our resources facilitate a smooth transition for both clients and referrers into our healthcare system. Throughout the entire admission, treatment, and discharge process, these resources are made available. Our professional team removes the challenges that usually accommodate the healthcare referral process.
Our Facilities and Admission
Quite often, regulatory processes can slow down the admissions process. We understand the process and can provide access to professionals, referring individuals, and organization with exact information and guidance to help clients comfortably transition into our therapy programs and treatment centers. If treatment is provided in one of our facilities, we conduct a thorough collaboration with referring health care professionals to gather as much pertinent medical information as possible.
This allows us to develop an effective treatment plan that addresses the client’s needs, and status of health. We are also very careful to expedite the process while maintaining our legal and ethical integrity. Our patients can also rest assured their information will remain confidential, as we adhere to the specific requirements mandated by the Health Insurance Portability and Accountability Act (HIPAA).
In an effort to further ensure adherence to required ethical and legal HIPAA guidelines, we request clients to authorize disclosure of their personal health information. On occasion, the referring organization may request updates on our client’s progress. There may be specific information that cannot be shared without the client’s consent. Only with a patient’s consent will our referral coordinator act as a liaison and provide the referrer with up-to-date information regarding the patient’s progress.
All authorized parties are privy to our client’s discharge.
